How to configure the "Menu item"

I will like to know how to configure this menu item.

and how to add the titles to the "contact form fields" like you have.

we added module(s) inside menu, it's quite easy, we have manual how to do this in general : http://www.gavick.com/documentation/gen ... into-menu/

In this case we added: News Show Pro GK5 module

I followed all the steps and it works to show the "Module in Menu position" but Im still having problems with the columns. Even if I change the columns number in the "Main menu item" still show me just 1 column.
