"Translated" Contact Page not showing email or name

Hello,
I seem to have an issue with the "Contact Form" page.

.. and this one is created through WPML: http://hopandup.com/en/get-in-touch-with-us/ that doesn´t show the email or name labels and fields.

Could you please provide me with a URL to your website and back-end access via PM (click the “Private Message” text underneath my avatar) so that I may analyze it? It is a lot easier for us to diagnose issues when we have a live site to examine.

On the right column of your contact page, there's "Page additional params" metabox, where you can disable/enable elements like e-mail, name fileds and on the english version these options were disabled.
