Notice: Undefined index: QUERY_STRING when trying to install

When I tried to install the quickstart for Corporate 2, above the entire installation process was the error:

Notice: Undefined index: QUERY_STRING in /hermes/web04/b1871/moo.username/Joomla3/libraries/joomla/environment/uri.php on line 161

"username" is my actual database username

I was not able to install the quickstart because it returned the error:

Unable to connect to the database:Could not connect to MySQL

Not sure if these issues are related or not. I contacted my host to see if they could reset the database connection to see if that might help.
