Quiz | Scoring test

Dear colleagues! Please help me with the following: I need to make a coll and UX\UI friendly asessment (grammar) test. User story is the following:
1. User should submit a form (enter his phone and e-mail)
2. He passes a test limited in time (it's better that each section limited in time)
3. The person sees his results (good\bad\satisfactory\ e.t.c)
4. I get on webmail (or CRM) phone, e-mail and results of a person.

What plugin or service would you suggest?!

I do not know of any extension capable of doing that. You may need to hire a developer to make such think for You. But first check in joomla extensions database: http://extensions.joomla.org/

Unfortunately surveymonkey doesn't allow to make quizes. Doeas anyone have any experience of the usage of such srvices?
