Issue with Mail Poet Subscription Form

Hello.

I've got an issue with the Mail Poet Subscription Form after installing the quickstart package. The form isn't positioned correctly on frontpage - moved down a lot. Also in back-end (MailPoet-Configuration) there're notifications on plenty of database errors.
Please see screenshots attached. Kindly advise how to fix it.

Could you please provide me with a URL to your website and back-end access via PM (click the “Private Message” text underneath my avatar) so that I may analyze it?

We've solved the issue.
For those who has the same problem, please find an option of reinstalling the plugin in 'MailPoet -- Settings -- Advanced -- Geeky Options'. It will recreate related database and fix errors. Then make a new form and assign it to the widget using tutorial: https://www.gavick.com/wordpress-themes ... ng-widgets.
