I'm a bit baffled by the events. It says in the docs:

Click on the "Screen Options" button on the top-right of the screen to bring up a list of viewable sections for the post screen

There is no 'Screen Options' button?

Also what are the 'extra fields' fields forks & gears for?

Most probably you have read the documentation for WordPress theme not Joomla template.

No I am reading the instructions from this website on this exact template:

"Now we need to make sure that we can add Custom Fields. Click on the "Screen Options" button on the top-right of the screen to bring up a list of viewable sections for the post screen; add a tick to the "Custom Fields" option. "

There is no 'Screen Options' in my admin panel

Are you using joomla or wordpress version of the template? If you are using a joomla version then you are reading wrong tutorial - because the linked article is for wordpress and contains screenshots from the wordpress back-end.
