Request Information Form and GK Contact Plugin

Hello. I am trying to get to work the requested information form displayed on the main page. I tried looking up the documentation, but it's not too clear how to set it up. I saw that University came with GK Contact plugin. Can they both work together? If yes, how can I accomplish this? If not, can you please point me to the right direction? I need a very simple contact form. Can I use the template form with built in Joomla contacts? Thank you.

1) GK contact plugin is a plugin it means it can be used in Custom HTML modules in any template
2) But If you're demanding customer please use contact module, on JED there are over 20 ready solutions also with custom fields.
Be a child again. Joomla is like a Lego.

But about contact appearance , use your CSS skills I hope you have them.
