Contact form

Hi, can you tell me how can I put a conntact form (https://demo.gavick.com/joomla25/storefront/) into the home page, for example bellow module "from our blog", and secondly is it possible to put a contact form on the left side and on the right side short text information like in Steakhouse template (https://demo.gavick.com/joomla25/steakhouse/).
