Linking a Menu title to module

Hi,

I want to link the Contact Form module (Do Not Hesitate to Contact Me) to a Menu named Contact Us (in Subpages).

I created and published the Menu link. Following are the details set:
Menu Item Type: External Link

Menu Params (GavickPro): Show Title - Yes;
Columns - 1;
Group - No;
Subcontent - Module position;
Select Positions - menu 1 (I've assigned the same position to the module as well);
Enable to Follow - No.

The module has been linked to this menu item and the position is also correct, but when I save it and refresh the page; the created Menu title doesn't appear with the other menu items.

Am I doing anything wrong / have I missed anything?

The important thing is that you want to link from subpages menu to homepage module. To do so you need to link to:
