to answer to your question I need to know what kind of module or plugin you/or somebody had use to build this form.
Because not all have feature to add new fields.
For example our simple "System - GK Contact" can not.

I have check the source code of this page you've added and I saw "marks" from www.wufoo.com - creator.

But I suppose that if better for you to use module or component with module.

Increasing the width of the template & using Menu Manager, hmm?

Or adding sub-items.... I'm not sure what kind of answer/solution you would like to hear
