How to change place in contact form ?

Hi,
in my site - http://www.brzozowiana.pl in contact i have info about chief editor

http://brzozowiana.pl/kontakt/9-redakcja-portalu/2-redakcja.html

but there is something wrong - in BA i fill like description show - for eg. name thant name, phone thant phone.... etc

but in front it display strange...
can we change it like in picture below ???


i would like to have all this information 1 by 1
so in 1st line : position
in 2nd line... city
in 3rd ZIP Code
etc...

I have one good tip:
Add all address info in one, for example street, field and you will be happy.

You can also use description field in Contact , you have so many options. Use which are practice and works for you.

i try this... but it place all information in 1 line and i wat to all information in new line :
in new line info about person
in new line info about city
in new about zip code
etc...

pls tell me only which file i should to change and i do it mysealf if you do not want to help me...
