footer menu items

I am trying to make the footer look like the image below:
i would like to use footer menu items but i do not know how to connect them..

at the contact us item how can i have a message form with options ie. type of enquiry:press, sales etc?

how can i reduce the height of the footer?

in the event where i manage to connect the footer items where will they appear? will they appear in the position i have placed on my image?? if not how can i do that?

Hi,
to publish items of your menu in footer just publish Menu module in footer_nav position and set your footer menu. About footer height issue please give me url to your website.

Hi Konrad, thank you for your response.
I know that i have to publish the footer module and i did it prior posting here, but nothing appears.. so how can i get the footer menu items to appear.

Also what about my question regarding the contact us form? i would like something similar to gavick site "contact" where you are given a selection of departments that you may wish to contact.

I am developing on MAMP so i cannot give you the link.. any alternative way?

Apparently there is! For the benefit of the other users i will copy paste the response i was given by teitbite:

To reduce footer please play with values of margin in this class:

#gkFooter {
margin: 32px auto 100px !important;
}

Your reply about the footer appearance was not accurate either. And you have a tendency to ignore part of my posts. Disapointing
