When I change, the “Banner placement” to “Top” there is no change on the website. The “Privacy settings” still show in the right bottom.
How do I change it to the same style used on gavick.com ??

It seems that there is something missing on your site, because with default installation banner shows on top with "Banner placement" "top"

It's a browser problem with "Internet Explorer" and "Mozilla Firefox". When using "Google Chrome" I can see the banner.

With FireFox it is still ok:
