Avatar upload stopped working

Hi,
The avatar upload has worked only with the super admin's account once and since then it has not worked. No errors or nothing. When you try to change the avatar regardless of which page or spot you are trying to change it from, it will act like it has uploaded something but nothing will become saved nor will it show to crop the image, etc.
Can you please help?

I have tried to upload the image from the following page(URL enclosed) and saw the error message again shown below:

Please switch to default template and check if problem still exist ?
