Request Information setup - University template

I am new to your templates and need some help with the module set up.
I need to use it as a "Send us your question" form.
There will be Your name, Your Question and Your email address labels which is no a problem for me.
I struggle with <form action="#"> piece.
Could please somebody give me some step by step instructions (or links) how to generate 'mailto' code to replace the '#' ?

I installed MailChimp component to the website but I am opened to other solutions.

We do not have an extension capable of doing that, but please check http://extensions.joomla.org/ site. I think there is a couple of such extensions. I remeber using RSForm in such exact manner some time ago.
