Rogue Banner Ad

I am building a new website, see: http://new411-mag-30.cloudaccess.net/.

I added an Amazon Skyscraper banner ad to the sidebar position for the Author page. However, the ad shows up in two places in the sidebar on the Authors page and a position I can't identify on every page as though it is an article. I tried turning the ad in the sidebar position from showing on any page. The result is the ad stops showing in the sidebar position, as it should. However, the ad still shows up as though it is an article on every page.

I cannot figure out what is causing this ad to show up in a position that I did not place it in.

Please advise.
