Boutique - Quick Start Set Up

Hello,

Can someone please give me some advice. I have uploaded the Boutique quick start package and now I am at the stage where I can see the Joomla! installation page when I visit my websites URL. However, when I get to "Stage 4 - Database" - its asks me for a username, database name and password. No matter what I do, I can not get this to work.

I value any help and suggestions please...
