Want some custom fields on title overlay

First, thank's for your pretty (M)Social theme !
The question is that I want put some custom fields under the overlay title like the screen below made on Photoshop.
How can I do ?

the widget is "GK Grid Title Overlay"
- I need custom fields appear without the title field
- n span tags as custom fields
- the title of the widget in a div or span
How do you do for payment?"

But have no response untill,
