on another it's ok

Your template is based on mootools 1.1 support. So, probably there's the problem, since our extension also work on mootools.
On advanced parameters of the module, try "force disable" option on "Use Mootools".

If this not fix, there's also the possibility of conflict with other 3th party extension script.

Well, if you want to use our extension, you need to get rid of the script that is in conflict with our.
